Esto se debe, en parte, al alto costo de los medicamentos. En el 2010 una familia promedio gast\u00f3 el 1,8 por ciento de su presupuesto familiar mensual total en medicamentos, seg\u00fan el Ministerio de Econom\u00eda. Esto corresponde al 60% de su gasto total en salud, una importante proporci\u00f3n que el Instituto de Salud P\u00fablica (ISP) del pa\u00eds aspira a reducir con la introducci\u00f3n de f\u00e1rmacos bioequivalentes.<\/p>\n<p style=\"text-align: justify;\">En la actualidad hay solo 70 medicamentos bioequivalentes aprobados en Chile, pero el ISP y la reci\u00e9n creada Agencia Nacional de Medicamentos (Anamed), el regulador del sector, planean incrementar esta cantidad a cerca de 236 para fines de este a\u00f1o.<\/p>\n<p style=\"text-align: justify;\">Un f\u00e1rmaco bioequivalente es una copia de un innovador f\u00e1rmaco original que contiene el mismo principio activo. Este ingrediente opera a la misma tasa molecular y concentraci\u00f3n en el torrente sangu\u00edneo que el original para conseguir el mismo efecto terap\u00e9utico, seg\u00fan el ISP, que basa su definici\u00f3n en la de la Organizaci\u00f3n Mundial de la Salud (OMS).<\/p>\n<p style=\"text-align: justify;\">Los medicamentos bioequivalentes pueden ser entre 5 y 20 veces m\u00e1s baratos que el original. Por ejemplo Zotran, un f\u00e1rmaco utilizado para ayudar a aliviar la ansiedad, cuesta 11.540 pesos (cerca de US$24) en Chile si se vende como un producto de marca. La versi\u00f3n bioequivalente del mismo f\u00e1rmaco se vende por 540 pesos, poco m\u00e1s de US$1, un ahorro del 95,3 por ciento.<\/p>\n<p style=\"text-align: justify;\">Una vez que la patente de un f\u00e1rmaco original expira, otras empresas pueden producir f\u00e1rmacos bioequivalentes a precios m\u00e1s bajos, sin necesidad de recuperar los costos financieros en los que ya incurri\u00f3 el fabricante original por la investigaci\u00f3n, el desarrollo y las pruebas. Esto introduce competencia al mercado, lo que hace disminuir los precios minoristas de los medicamentos para los consumidores y mejora el acceso al producto.<\/p>\n<p style=\"text-align: justify;\"><strong>Medicamentos Accesibles<\/strong><\/p>\n<p style=\"text-align: justify;\">El esquema ha sido bien recibido por los chilenos, quienes -a diferencia de pa\u00edses con sistemas de salud p\u00fablicos y gratuitos como el Reino Unido- tienen que pagar por los medicamentos. Tambi\u00e9n ayudar\u00e1 a asegurar el suministro de medicamentos en el sistema p\u00fablico de salud y, a su vez, generar m\u00e1s ahorros para los pacientes; algo que se necesita con urgencia a juicio del Dr. Enrique Paris, presidente del Colegio M\u00e9dico de Chile.<img loading=\"lazy\" decoding=\"async\" style=\"float: left; margin: 4px;\" src=\"\/sites\/default\/files\/Enrique_Paris_1.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\">En Chile, el 90 por ciento de los centros primarios de salud son operados por municipios, no por el Estado. En los \u00faltimos a\u00f1os, la mala administraci\u00f3n del sistema tuvo como resultado una \u201ccadena de deudas\u201d entre estos centros y los laboratorios que los abastecen, lo que en definitiva ha hecho que los pacientes terminen pagando la cuenta. Pese a que el 80 por ciento de las personas en Chile tiene acceso a medicamentos gratuitos en virtud del Fondo Nacional de Salud (FONASA), el sistema de seguro de salud p\u00fablico, muchas veces ocurre que los f\u00e1rmacos necesarios no est\u00e1n en existencia.<\/p>\n<p style=\"text-align: justify;\">\u201c\u00bfQu\u00e9 hace la persona entonces?\u201d, pregunta Paris. \u201cVa a comprar el remedio a la farmacia de la esquina y gasta su propio dinero. Esta es la raz\u00f3n por la que el gasto de las personas en medicamentos es muy alto\u201d.<\/p>\n<p style=\"text-align: justify;\">El plan de bioequivalencia tambi\u00e9n dejar\u00e1 las finanzas del Gobierno en un estado m\u00e1s saludable. Si un medicamento bioequivalente, en promedio, cuesta un tercio del precio de su original, se pueden comprar tres veces m\u00e1s, aumentando as\u00ed las existencias, mejorando el acceso y ahorrando dinero fiscal. Adem\u00e1s, Cenabast -la agencia de gobierno que suministra medicamentos al sistema p\u00fablico de salud- recibe un descuento adicional del 30 por ciento de los laboratorios, lo que significa que el precio de los remedios podr\u00eda caer en a lo menos un 50 a un 60 por ciento.<\/p>\n<p style=\"text-align: justify;\">Seg\u00fan Jaime Ma\u00f1alich, el ministro de Salud de Chile, el Gobierno del presidente Sebasti\u00e1n Pi\u00f1era ha reducido la cantidad adeudada a los laboratorios poo Cenabast de US$160 millones a US$70 millones y aspira a pagar el resto para fines de a\u00f1o, dejando al sistema p\u00fablico libre para realizar aquello para lo que fue creado: proporcionar cuidado de la salud y medicamentos a la gente.<\/p>\n<p style=\"text-align: justify;\">\u201cEstamos avanzando muy r\u00e1pidamente en direcci\u00f3n a hacer que los medicamentos sean parte de la garant\u00eda de salud\u201d, sostuvo Ma\u00f1alich en una conferencia de prensa celebrada para coincidir con la visita a Chile del subdirector general de la OMS, Keiji Fukuda, quien calific\u00f3 el planteamiento del Gobierno sobre el tema como s\u00f3lido y orientado al futuro.&nbsp;<img loading=\"lazy\" decoding=\"async\" style=\"float: right; margin: 4px;\" src=\"\/sites\/default\/files\/MINISTRO_DE_SALUD_1.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\"><strong>Beneficios para la Salud<\/strong><\/p>\n<p style=\"text-align: justify;\">No obstante, los medicamento bioequivalentes no tienen que ver solo con ahorrar dinero, de hecho estudios muestran que los ahorros generados por los f\u00e1rmacos bioequivalentes alcanzan un estancamiento despu\u00e9s de la primera ca\u00edda inicial del precio.<\/p>\n<p style=\"text-align: justify;\">En cambio, \u201clos medicamento bioequivalentes tienen que ver con la econom\u00eda en general y con mejorar la salud\u201d, sostiene el doctor Iv\u00e1n Saavedra, director del Centro de Bioequivalencia de la Universidad de Chile, uno de los 10 laboratorios que actualmente someten a prueba los f\u00e1rmacos para confirmar su bioequivalencia en Chile.<\/p>\n<p style=\"text-align: justify;\">A diferencia de los f\u00e1rmacos gen\u00e9ricos, los medicamento bioequivalentes necesitan ser certificados antes de que su venta pueda ser aprobada: una larga y costosa tarea que en Chile le corresponde al ISP. Pero una vez que se ha finalizado esta labor, los pacientes pueden estar seguros de que el f\u00e1rmaco que est\u00e1n consumiendo es exactamente lo que dice que es.<\/p>\n<p style=\"text-align: justify;\">Este no era el caso previamente. Cuando la cruzada por alternativas m\u00e9dicas m\u00e1s baratas comenz\u00f3 en Chile durante la presidencia de Eduardo Frei en 1964, los f\u00e1rmacos que se introdujeron fueron gen\u00e9ricos no bioequivalentes. Los f\u00e1rmacos gen\u00e9ricos, como los bioequivalentes, son m\u00e1s baratos y cumplen m\u00e1s o menos la misma funci\u00f3n m\u00e9dica que sus antecesores patentados, pero no son id\u00e9nticos a nivel molecular. Esto hace que sea peligroso para doctores y farmac\u00e9uticos calcular la dosis apropiada al prescribirlos y venderlos, destaca Saavedra.<\/p>\n<p style=\"text-align: justify;\">Los f\u00e1rmacos gen\u00e9ricos a\u00fan corresponden al 38 por ciento de las ventas de la industria en Chile, pero el sistema de certificaci\u00f3n da a los f\u00e1rmacos bioequivalentes un \u201cextra\u201d, sostiene H\u00e9ctor Rojas, presidente de la Asociaci\u00f3n de Farmac\u00e9uticos Independientes. \u201cSi el remedio es bueno para los pacientes, es bueno para mi farmacia\u201d, sostiene. No obstante, lograr la confianza de los clientes, para alentarlos a comprar medicamentos nuevos, es uno de los mayores desaf\u00edos que enfrenta la iniciativa.<\/p>\n<p style=\"text-align: justify;\"><strong>Confiar en la Bioequivalencia<\/strong><\/p>\n<p style=\"text-align: justify;\">No es ninguna novedad que la gente lo piensa dos veces antes de ingerir nuevos planes farmac\u00e9uticos. En marzo del a\u00f1o pasado un esc\u00e1ndalo de fijaci\u00f3n de precios revel\u00f3 que los chilenos hab\u00edan sido enga\u00f1ados por las tres principales farmacias del pa\u00eds.<img loading=\"lazy\" decoding=\"async\" style=\"float: left; margin: 4px;\" src=\"\/sites\/default\/files\/IVAN_SAAVEDRA_1.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\">Luego, a comienzos de este a\u00f1o, los medios de comunicaci\u00f3n llevaron la atenci\u00f3n a los incentivos econ\u00f3micos que persuad\u00edan a los trabajadores de las farmacias a recomendar algunos f\u00e1rmacos por sobre otros. Incluso la pr\u00e1ctica de mantener productos inofensivos como maquillaje en muestrarios fomenta un ambiente de desconfianza.<\/p>\n<p style=\"text-align: justify;\">Adem\u00e1s, los chilenos creen de manera inquebrantable en la calidad de los productos de marca. \u201cEs muy sicol\u00f3gico. Aqu\u00ed en Chile, la gente cree que las cosas baratas son malas. Quieren el Rolls Royce de los productos\u201d, afirma Paris.<\/p>\n<p style=\"text-align: justify;\">Afortunadamente, los medicamentos bioequivalentes recibir\u00e1n sus propias marcas. Estos f\u00e1rmacos tendr\u00e1n envases con una banda amarillas brillante y la palabra \u201cbioequivalente\u201d en rojo. Adem\u00e1s los f\u00e1rmacos tambi\u00e9n mantendr\u00e1n su nombre gen\u00e9rico, conocido como Denominaci\u00f3n Com\u00fan Internacional o no patentable, lo que permite a los consumidores reconocer el producto en cualquier parte del mundo.<\/p>\n<p style=\"text-align: justify;\">Entonces \u00bfpor qu\u00e9 si los medicamentos bioequivalentes son sanos, baratos y han estado presentes en la mayor\u00eda de los pa\u00edses desarrollados por d\u00e9cadas, su introducci\u00f3n en el mercado chileno es tan controversial?<\/p>\n<p style=\"text-align: justify;\">\u201cChile es nuevo en esto\u201d, afirma Rojas. \u201cSomos m\u00e1s retr\u00f3grados que los pa\u00edses europeos, Estados Unidos, Brasil, Canad\u00e1 y Jap\u00f3n, que inmediatamente comenzaron a aceptar las recomendaciones de la OMS con respecto a la calidad, seguridad y eficacia de los medicamentos a fines de la d\u00e9cada de los 80\u201d.<\/p>\n<p style=\"text-align: justify;\">En muchas maneras es admirable que el Gobierno siga avanzando con pol\u00edticas de bioequivalencia: el ministro Ma\u00f1alich ha prometido que para fines del 2014 cada medicamento gen\u00e9rico en Chile tambi\u00e9n \u201ctendr\u00e1 el sello de bioequivalencia\u201d. Pero existen algunos en la profesi\u00f3n m\u00e9dica y en las empresas farmac\u00e9uticas que creen Chile est\u00e1 tratando de correr antes de poder caminar.<\/p>\n<p style=\"text-align: justify;\"><strong>Pr\u00e1cticas de Fabricaci\u00f3n Certificadas<\/strong><\/p>\n<p style=\"text-align: justify;\">Desde un punto de vista t\u00e9cnico solo el 40% de los laboratorios en Chile est\u00e1n certificados con Buenas Pr\u00e1cticas de Manufactura, que es una desventaja significativa para un esquema que requiere que los laboratorios demuestren la seguridad y eficacia de nuevos f\u00e1rmacos.<\/p>\n<p style=\"text-align: justify;\">\u201cLas Buenas Pr\u00e1cticas de Manufactura son el primer piso del edificio y el segundo es la bioequivalencia. Obviamente no vamos a prescribir medicamentos que son bioequivalentes si no demuestran tambi\u00e9n buenas pr\u00e1cticas\u201d, afirma Paris.<\/p>\n<p style=\"text-align: justify;\">Esta preocupaci\u00f3n es compartida por la gerente general de Pfizer Chile, M\u00f3nica Zerpa. \u201cNuestra principal preocupaci\u00f3n con esta pol\u00edtica es c\u00f3mo la bioequivalencia cumplir\u00e1 con los est\u00e1ndares farmac\u00e9uticos internacionales y que los laboratorios no reduzcan gastos que afecten la calidad para un cumplimiento m\u00e1s r\u00e1pido de los requerimientos b\u00e1sicos\u201d.<\/p>\n<p style=\"text-align: justify;\">No obstante, muchos optan por ver lo positivo. Con un mayor enfoque en la manufactura y certificaci\u00f3n de f\u00e1rmacos bioequivalentes, el ISP podr\u00eda verse obligado a examinar con m\u00e1s detalle las pr\u00e1cticas de manufactura.<\/p>\n<p style=\"text-align: justify;\">\u201cTodo el programa de bioequivalencia es una medida de seguridad\u201d, dice Fukuda de la OMS. \u201cTodo este esfuerzo apunta realmente a incrementar la revisi\u00f3n y asegurar tanto la calidad como la seguridad de los f\u00e1rmacos que se est\u00e1n utilizando\u201d.&nbsp;<img loading=\"lazy\" decoding=\"async\" style=\"float: right; margin: 4px;\" src=\"\/sites\/default\/files\/HECTOS_ROJAS_1.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\">Culturalmente adem\u00e1s las pol\u00edticas del Ministerio de incrementar el acceso podr\u00edan hacer m\u00e1s da\u00f1o que beneficio. Estos planes, que est\u00e1n incluyen un aumento en la cantidad de farmacias que atienden las 24 horas del d\u00eda todos los d\u00edas de la semana y la introducci\u00f3n de una flota de farmacias m\u00f3viles, tambi\u00e9n incluye un proyecto de ley para legalizar la venta de medicamentos en lugares como supermercados, un proyecto de ley que Rojas ha denunciado como \u201ccriminal\u201d.<\/p>\n<p style=\"text-align: justify;\">Ma\u00f1alich afirma que el proyecto de ley mejorar\u00e1 el acceso, especialmente en las 50 de las 320 municipalidades de Chile que no tienen una sola farmacia, pero Rojas se opone a la idea de vender medicamentos \u201ccomo dulces\u201d en sitios que no sean farmacias. Menciona como ejemplo a Argentina, naci\u00f3n que intent\u00f3 algo similar en 1991, solo para revertir la decisi\u00f3n casi dos d\u00e9cadas despu\u00e9s tras problemas de automedicaci\u00f3n.<\/p>\n<p style=\"text-align: justify;\">Uno de los temores respecto a hacer que los f\u00e1rmacos est\u00e9n disponibles en cada minimarket, incluso los aparentemente benignos como el paracetamol -un f\u00e1rmaco que accidentalmente cobra la vida de alrededor de 900 chilenos al a\u00f1o\u2013, son que el nivel de conciencia de la gente sobre los medicamentos a\u00fan no est\u00e1 suficientemente desarrollado.<\/p>\n<p style=\"text-align: justify;\">\u201cEl beneficio de las farmacias es que el consejo es gratis y uno no tendr\u00e1 eso en una tienda com\u00fan\u201d, sostiene Rojas.<\/p>\n<p style=\"text-align: justify;\">La bioequivalencia tiene que ver con mejorar el acceso a f\u00e1rmacos, un proceso largo y agotador de pruebas y certificaci\u00f3n, pero uno que abre opciones m\u00e1s costeables para el paciente. Tal como ha ocurrido con muchos avances cient\u00edficos y sociales, cambios como este a menudo generan m\u00e1s preguntas que respuestas, pero si se realizan de manera adecuada los chilenos finalmente podr\u00e1n conseguir algo que supuestamente el dinero no puede comprar: la salud.<\/p>\n<p style=\"text-align: justify;\"><strong>Olivia Crellin trabaja como periodista freelance en Santiago<\/strong><\/p>\n<p><!--:--><!--:en--><\/p>\n<p style=\"text-align: justify;\">In Chile healthcare, like education, is expensive. This is, in part, due to the high cost of medication. In 2010 the average family spent 1.8 percent of their total monthly household budget on medicines, according to the Ministry of Economy. This represents 60% of their total spending on healthcare, which is a hefty proportion that the country\u2019s Institute of Public Health (ISP) aims to reduce with the introduction of bioequivalent drugs.<\/p>\n<p style=\"text-align: justify;\">Currently there are only 70 approved bioequivalent drugs in Chile<strong> <\/strong>but the ISP and the newly created national drug regulator, Anamed, plan to increase this to around 236 by the end of the year.<\/p>\n<p style=\"text-align: justify;\">A bioequivalent drug is \u201ca copy of an original innovator drug which contains the same active ingredient. This ingredient operates at the same molecular rate and concentration in the bloodstream as the original to achieve an identical therapeutic effect,\u201d according to the ISP, which bases its definition on that of the World Health Organization (WHO).<\/p>\n<p style=\"text-align: justify;\">Bioequivalent drugs can be between five and 20 times cheaper than the original. For example Zotran, a drug used to help alleviate anxiety, costs 11,540 pesos (around US$24) in Chile if sold as a branded product. The bioequivalent version of the same drug sells for 540 pesos, just over US$1, a savings of 95.3 percent.<\/p>\n<p style=\"text-align: justify;\">Once the patent on an original drug has expired, other companies can produce bioequivalent drugs more cheaply, not needing to recoup the financial costs already incurred by the original manufacturer for research, development and testing. This introduces competition into the market, driving down the retail price of the drug for consumers and enhancing access to the product.<\/p>\n<p style=\"text-align: justify;\"><strong>Affordable medication<\/strong><\/p>\n<p style=\"text-align: justify;\">The scheme has been welcomed by Chileans, who, unlike in countries with free public healthcare such as the U.K., have to pay for medicine. It will also help ensure the supply of medicine in the public health system and, in turn, generate further savings for patients; something urgently needed according to Dr. Enrique Paris, president of the Chilean College of Physicians.&nbsp;<img loading=\"lazy\" decoding=\"async\" style=\"float: left; margin: 4px;\" src=\"\/sites\/default\/files\/Enrique_Paris_2.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\">In Chile 90 percent of primary healthcare clinics are operated by municipalities, not the state. In recent years the mismanagement of this system has resulted in a \u201cchain of debt\u201d between clinics and the laboratories that supply them, which has ultimately left the patient to foot the bill. Despite 80 percent of people in Chile having access to free drugs under FONASA, the public health insurance system, there are many times when the necessary drugs are not in stock.<\/p>\n<p style=\"text-align: justify;\">\u201cWhat does a person do then?\u201d Paris asks. \u201cThey go to buy the medicine at the pharmacy on the corner, and spend their own money. This is why the public\u2019s spending on medicine is very high.\u201d<\/p>\n<p style=\"text-align: justify;\">The bioequivalence plan will leave government finances in a healthier state too. If, on average, a bioequivalent drug costs a third of the price of its original, three times as much can be purchased, thereby enlarging stock, improving access and saving the public money. In addition, Cenabast, the government agency that supplies medicines to the public health system, receives an additional 30 percent discount from laboratories, which means that the price of drugs could fall by at least 50 to 60 percent.<\/p>\n<p style=\"text-align: justify;\">According to Jaime Ma\u00f1alich, Chile\u2019s Health Minister, President Pi\u00f1era\u2019s administration has reduced the amount owed to laboratories by Cenabast from US$160 million to US$70 million and aims to pay off the rest by the end of the year, leaving the public health system free to do what it was created for \u2013 provide healthcare and medicines to the public.<\/p>\n<p style=\"text-align: justify;\">\u201cWe are moving very fast in the direction of making drugs part of the health guarantee,\u201d Ma\u00f1alich said at a press conference held to coincide<strong><em> <\/em><\/strong>with the visit to Chile of WHO Assistant Director-General Keiji Fukuda, who considered the government\u2019s thinking on the matter \u201cfuture-orientated and solid\u201d.<img loading=\"lazy\" decoding=\"async\" style=\"float: right; margin: 4px;\" src=\"\/sites\/default\/files\/MINISTRO_DE_SALUD_2.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\"><strong>Health benefits<\/strong><\/p>\n<p style=\"text-align: justify;\">Bioequivalent drugs are not just about saving money though \u2013 in fact studies show that the savings generated by bioequivalent drugs plateau after the first initial drop in price.<\/p>\n<p style=\"text-align: justify;\">Instead, \u201cbioequivalent drugs are about economy in general and about improving health,\u201d says Dr. Iv\u00e1n Saavedra, director of the Center of Equivalence at the University of Chile, one of ten laboratories that currently test drugs for their bioequivalence in Chile.<\/p>\n<p style=\"text-align: justify;\">Unlike generic drugs, bioequivalent drugs need to be certified before they can be approved for sale; a long and costly undertaking that in Chile falls to the ISP. But once this task has been completed, patients can be sure that the drug they are consuming is exactly what it says it is.<\/p>\n<p style=\"text-align: justify;\">This was not the case previously. When the quest for cheaper medical alternatives began in Chile during the presidency of Eduardo Frei in 1964, the drugs that were introduced were generic, not bioequivalent. Generic drugs, like bioequivalent drugs, are cheaper and fulfill the same medical function more or less as their patented ancestors, but they are not identical on a molecular level. This makes it dangerous for doctors and pharmacists to calculate the appropriate dosage when prescribing and selling them, points out Saavedra.<\/p>\n<p style=\"text-align: justify;\">Generic drugs still account for 38 percent of industry sales in Chile, but the system of certification gives bioequivalent drugs an \u201cextra\u201d, says Hector Rojas, the president of the Association of Independent Pharmacists. \u201cIf the drug is good for patients, it\u2019s good for my pharmacy,\u201d he says. But securing the confidence of customers, to encourage them to buy the new drugs, is one of the biggest challenges facing the initiative.<\/p>\n<p style=\"text-align: justify;\"><strong>Trusting in bioequivalence<\/strong><\/p>\n<p style=\"text-align: justify;\">It is no wonder that the public thinks twice before swallowing new pharmaceutical schemes. In March last year a price-fixing scandal revealed that Chileans had been cheated by the country\u2019s three biggest pharmacies: Salcobrand, Farmacias Ahumada and Cruz Verde.<img loading=\"lazy\" decoding=\"async\" style=\"float: left; margin: 4px;\" src=\"\/sites\/default\/files\/IVAN_SAAVEDRA_2.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\">Then, earlier this year, the media brought attention to economic incentives that persuade pharmacy workers to recommend some drugs over others. Even the practice of keeping harmless products like make-up in display cabinets fosters an environment of mistrust.<\/p>\n<p style=\"text-align: justify;\">In addition, Chileans have an unshakable belief in the quality of branded products. \u201cIt is very psychological. Here in Chile, people think that cheap things are bad. They want the Rolls Royce of products,\u201d says Paris.&nbsp;<\/p>\n<p style=\"text-align: justify;\">Luckily, Chile\u2019s bioequivalent drugs will be getting a branding all of their own. The drugs will come in packaging with a bright yellow band and the word \u201cBioequivalent\u201d in red. The drugs will also keep their generic name, known as the International Nonproprietary Name, which allows customers to recognize the product anywhere in the world.<\/p>\n<p style=\"text-align: justify;\">So if bioequivalent drugs are healthy, cheap and have been around in most developed countries for decades, why is their introduction into the Chilean market so controversial?<\/p>\n<p style=\"text-align: justify;\">\u201cChile is new to this,\u201d says Rojas. \u201cWe are more backward than the European countries, the U.S., Brazil, Canada and Japan, which immediately started to accept the recommendations of the WHO with regards to the quality, safety and efficacy of medicines in the late 1980s.\u201d<\/p>\n<p style=\"text-align: justify;\">In many ways it is admirable that the government is racing ahead with bioequivalence policies &#8211; Ma\u00f1alich has promised that by the end of 2014 every generic drug in Chile \u201cwill have the bioequivalence seal\u201d as well. But there are those in the medical profession and pharmaceutical business that think Chile is trying to run before it can walk.<\/p>\n<p style=\"text-align: justify;\"><strong>Certified manufacturing practices<\/strong><\/p>\n<p style=\"text-align: justify;\">From a technical standpoint only 40% of laboratories in Chile are fully GMP (Good Manufacturing Practice) certified, which is a significant drawback for a scheme that requires laboratories to prove the safety and efficacy of new drugs.<\/p>\n<p style=\"text-align: justify;\">\u201cGMP is the first floor of the building and the second is bioequivalence. Obviously we are not going to prescribe medicines that are bioequivalent if they don\u2019t also demonstrate good practices,\u201d says Paris.<\/p>\n<p style=\"text-align: justify;\">This concern is shared by Pfizer Chile\u2019s general manager, M\u00f3nica Zerpa. \u201cOur main concern with this policy is how bioequivalence will meet international pharmaceutical standards and that laboratories do not cut corners for a faster compliance with basic requirements.\u201d<\/p>\n<p style=\"text-align: justify;\">Many choose to see a silver lining, however. With an increased focus on the manufacture and certification of bioequivalent drugs, the ISP may be forced to scrutinize manufacturing practices more closely.<\/p>\n<p style=\"text-align: justify;\">\u201cThe whole bioequivalency program is a safety step,\u201d says the WHO\u2019s Fukuda. \u201cThis entire effort is really to increase scrutiny and ensure both the<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" style=\"float: right; margin: 4px;\" src=\"\/sites\/default\/files\/HECTOS_ROJAS_2.jpg\" alt=\"\" width=\"150\" height=\"200\"><\/p>\n<p style=\"text-align: justify;\">&nbsp;quality and the safety of the drugs being use.\u201d<\/p>\n<p style=\"text-align: justify;\">Culturally as well the Ministry\u2019s policies to increase access could risk doing more harm than good. These plans, which consist of increasing the number of 24\/7 pharmacies and introducing a fleet of mobile pharmacies, also include a bill to legalize the sale of drugs in places like supermarkets \u2013 a bill denounced by Rojas as \u201ccriminal\u201d.<\/p>\n<p style=\"text-align: justify;\">Ma\u00f1alich says the bill would improve access, especially in the 50 of Chile\u2019s 320 municipalities that don\u2019t have a single pharmacy, but Rojas is against the idea of selling medicines \u201clike candy\u201d in places other than pharmacies. He cites the example of Argentina which tried something similar in 1991, only to reverse the decision almost two decades later following problems with self-medication.&nbsp;<\/p>\n<p style=\"text-align: justify;\">One of the fears of making drugs available in every corner store, even seemingly mild ones like paracetamol \u2013 a drug that incidentally kills around 900 Chileans a year \u2013 is that levels of public awareness about medicine are not yet sufficiently developed.<\/p>\n<p style=\"text-align: justify;\">\u201cThe benefit of pharmacies is that advice is free and you won\u2019t get that in an ordinary shop,\u201d says Rojas.<\/p>\n<p style=\"text-align: justify;\">Bioequivalence is about improving access to pharmaceutical drugs, a grueling and lengthy process of testing and certification, but one that opens up more affordable options for the patient.
