[:es]Desde hace unos a\u00f1os, algunas cl\u00ednicas est\u00e1n trabajando con hospitales estadounidenses de primera l\u00ednea, tanto para capacitar a sus m\u00e9dicos como para desarrollar una labor conjunta en la atenci\u00f3n de pacientes. Incluso, capitales de ese pa\u00eds han entrado en la propiedad de centros de salud locales. El resultado, dicen en la industria, puede tener un fuerte impacto en la mejora de la atenci\u00f3n en Chile.<\/strong><\/p>\n

Hace algunas semanas se cumplieron 20 a\u00f1os de la firma de uno de los acuerdos de m\u00e1s larga data entre un centro de salud chileno y uno estadounidense. Se trata del suscrito por la Cl\u00ednica Alemana y Cl\u00ednica Mayo, hoy ubicada en el primer lugar del ranking anual de los Mejores Hospitales de U.S. News & World Report 2017-2018. A este convenio se suma el que firmaron en octubre pasado la Cl\u00ednica Universidad de los Andes y el Massachusetts General Hospital, que est\u00e1 en el cuarto lugar de la misma medici\u00f3n, y la entrada en 2013 de CHRISTUS Health a la propiedad de la Red de Salud UC.<\/p>\n

Movimientos que han sido inspirados en un \u00e1nimo de colaboraci\u00f3n, sobre la base de valores y visiones compartidas, y que han impactado directamente en los est\u00e1ndares de los centros de salud chilenos, con el consiguiente beneficio para los pacientes.<\/p>\n

Una situaci\u00f3n que el director comercial de la Cl\u00ednica Universidad de los Andes, Arturo Z\u00fa\u00f1iga, compara con el ingreso de Chile a la Organizaci\u00f3n para la Cooperaci\u00f3n y el Desarrollo Econ\u00f3mico (OCDE), cuando dej\u00f3 de medirse con los pa\u00edses latinoamericanos.<\/p>\n

En ese momento \u201cempezamos a ver c\u00f3mo estaba nuestra productividad, ingreso per c\u00e1pita, nivel educacional y una serie de otros indicadores en relaci\u00f3n a pa\u00edses que ya tienen mucho camino recorrido y, por tanto, mejores resultados. Haciendo el paralelismo, es muy bueno replicar las mejores pr\u00e1cticas de instituciones de salud que son referentes mundiales\u201d, explica.<\/p>\n

Para el gerente general de Sanofi, Henry Ordo\u00f1ez, Chile tiene las capacidades tecnol\u00f3gicas y profesionales para ofrecer una atenci\u00f3n por encima de sus pares a nivel de Latinoam\u00e9rica. Pero a\u00f1ade que existen procesos que pueden mejorarse para lograr una mejor eficiencia en la inversi\u00f3n de los recursos, as\u00ed como un seguimiento a los mismos buscando no s\u00f3lo la respuesta a corto plazo, sino tambi\u00e9n a mediano y largo alcance. Todo lo cual podr\u00eda enmarcarse en programas transversales que apunten a una mejor atenci\u00f3n y cobertura.<\/p>\n

As\u00ed, el aporte de este tipo de acuerdos, m\u00e1s all\u00e1 de posibles inversiones, est\u00e1 en la mejora de los procesos o en la estandarizaci\u00f3n de protocolos, que permiten tener mejores formas de comparar los est\u00e1ndares locales con los internacionales.<\/p>\n

\"\"<\/p>\n

Colaboraci\u00f3n consolidada<\/strong><\/p>\n

El convenio de colaboraci\u00f3n cient\u00edfico-docente que hace ya dos d\u00e9cadas firmaron Cl\u00ednica Alemana y Cl\u00ednica Mayo surgi\u00f3 del inter\u00e9s del establecimiento chileno por establecer v\u00ednculos con uno de los hospitales que, con m\u00e1s de 150 a\u00f1os de existencia, es uno de los m\u00e1s valorados en Estados Unidos.<\/p>\n

La coincidencia entre los valores y principios de ambos tambi\u00e9n motiv\u00f3 el acuerdo, precisa el jefe del Departamento de Medicina Interna de Cl\u00ednica Alemana, Dr. Luis Miguel Noriega: el paciente en el centro del quehacer de la instituci\u00f3n, que asume un deber en la educaci\u00f3n m\u00e9dica.<\/p>\n

\u201cEste intercambio de conocimiento y la b\u00fasqueda de la investigaci\u00f3n tienen un valor significativo y mutuo. Nosotros y nuestros colegas en Chile trabajamos para avanzar en la medicina en los Estados Unidos, Chile y en todo el mundo\u201d, comenta el Dr. Alfredo Qui\u00f1ones-Hinojosa, jefe de neurolog\u00eda de Cl\u00ednica Mayo en Jacksonville (Florida), y presidente m\u00e9dico para los proyectos e iniciativas en Latinoam\u00e9rica de la entidad.<\/p>\n

Aunque la relaci\u00f3n del establecimiento con Chile comenz\u00f3 cuando su cofundador, el Dr. William Mayo, visit\u00f3 el pa\u00eds y estableci\u00f3 relaciones con m\u00e9dicos y hospitales locales hace m\u00e1s de un siglo, la idea de este convenio en particular se gest\u00f3 en el marco de una visita que los directores de Cl\u00ednica Mayo hicieron a Latinoam\u00e9rica.<\/p>\n

En la oportunidad, el Dr. Claus Krebs \u2013director m\u00e9dico de Cl\u00ednica Alemana en ese tiempo\u2013, inici\u00f3 los contactos. A poco andar, y gracias al apoyo del Dr. Salvador \u00c1lvarez, entonces encargado de relaciones internacionales del centro estadounidense, el acercamiento culmin\u00f3 con este acuerdo, entre cuyas iniciativas se encuentra el programa Actualizaci\u00f3n del Board de Medicina Interna. Dicha acci\u00f3n se realiza desde el a\u00f1o 2005 en conjunto y bianualmente, con el fin de preparar a los m\u00e9dicos chilenos para el examen de certificaci\u00f3n y recertificaci\u00f3n en Estados Unidos.<\/p>\n

Los lazos institucionales y personales que se han ido creando a lo largo de 20 a\u00f1os de colaboraci\u00f3n han permitido, seg\u00fan el Dr. Noriega, que pacientes chilenos hayan sido evaluados por profesionales de la cl\u00ednica estadounidense como segunda opini\u00f3n, o hayan sido presentados a los<\/p>\n

equipos m\u00e9dicos de ese centro para obtener opiniones y sugerencias de manejo a nivel diagn\u00f3stico y terap\u00e9utico.<\/p>\n

A su vez, el equipo de Cl\u00ednica Alemana ha realizado visitas a Estados Unidos para conocer de cerca los modelos aplicados y ver su aplicabilidad en el medio local. Uno de los aspectos m\u00e1s destacados en este sentido fue el inicio de los registros electr\u00f3nicos en el establecimiento nacional, pues numerosos profesionales \u2013m\u00e9dicos y de otros rubros\u2013 pudieron conocer el funcionamiento de \u00e9stos en Cl\u00ednica Mayo, abordando ah\u00ed los problemas y bondades de su uso.<\/p>\n

\u201cCl\u00ednica Mayo ha abierto sus puertas para mostrarnos su modelo de atenci\u00f3n, sus l\u00edneas de desarrollo, sus errores y logros. Aspectos que han tenido un fuerte impacto en algunas decisiones que hemos tomado en nuestras planificaciones estrat\u00e9gicas\u201d, dice el jefe del Departamento de Medicina Interna de Cl\u00ednica Alemana.<\/p>\n

Segunda opini\u00f3n<\/strong><\/p>\n

Uno de los convenios m\u00e1s recientes suscritos por un centro de salud chileno y uno estadounidense es el que dieron a conocer en octubre la Cl\u00ednica Universidad de Los Andes y el Massachusetts General Hospital (Mass General), primer hospital cl\u00ednico de la Universidad de Harvard.<\/p>\n

Seg\u00fan el director comercial de Cl\u00ednica Universidad de los Andes, fueron representantes del Mass General quienes se acercaron al centro de salud nacional, coincidiendo en que ambos son campus cl\u00ednicos sin fines de lucro, donde la investigaci\u00f3n y la docencia son parte de sus prioridades.<\/p>\n

El acuerdo posibilita que pacientes chilenos puedan solicitar el env\u00edo de sus antecedentes a Boston para ser revisados por m\u00e9dicos del Mass General, a fin de obtener una segunda opini\u00f3n. En pocas semanas de funcionamiento, m\u00e1s de 10 pacientes con enfermedades complejas ya han solicitado este servicio y sus antecedentes m\u00e9dicos est\u00e1n en proceso de revisi\u00f3n por parte de un equipo multidisciplinario de especialistas, para determinar si se presentan en Estados Unidos.<\/p>\n

Para los especialistas chilenos, en tanto, el acuerdo les permite interactuar con los referentes mundiales de cada especialidad, compartiendo t\u00e9cnicas e intercambiando opiniones.<\/p>\n

\u201cEl mejor impacto que instituciones como Mass General pueden producir en Chile es fortalecer a\u00fan m\u00e1s sus sistemas de salud, junto con apoyar a los m\u00e9dicos para que brinden una excelente atenci\u00f3n a los pacientes a nivel local\u201d, se\u00f1ala la directora de Marketing y directora senior de Servicios de Salud Internacionales del Massachusetts General Hospital, Misty Hathaway.<\/p>\n

En su opini\u00f3n, Chile tiene un sistema de salud fuerte, con cl\u00ednicas y hospitales de alta calidad. Pero como en muchos pa\u00edses, dice, los chilenos experimentan una tasa de crecimiento de ciertas enfermedades \u2013c\u00e1ncer de est\u00f3mago y de pulm\u00f3n, trastornos del ri\u00f1\u00f3n, diabetes\u2013, afecciones que pueden relacionarse con el estilo de vida, la dieta y el ambiente, por lo que desde ese centro ven el acuerdo como una importante oportunidad para que los m\u00e9dicos chilenos eduquen mejor a las personas sobre los factores de riesgo. \u201cNing\u00fan sistema de salud en el mundo es perfecto, \u00a1incluyendo los Estados Unidos!\u201d, recalca.<\/p>\n

Innovaci\u00f3n y est\u00e1ndares<\/strong><\/p>\n

Las condiciones del pa\u00eds, su econom\u00eda estable y la oportunidad de tener un socio con los mismos valores y visi\u00f3n, fueron las razones que decidieron a CHRISTUS Health a escoger Chile, en el marco de su objetivo estrat\u00e9gico de expandirse a Latinoam\u00e9rica.<\/p>\n

El centro de salud de Texas, que pertenece a la Orden de las Hermanas de la Caridad del Verbo Encarnado y que ya contaba con presencia en M\u00e9xico, identific\u00f3 los pa\u00edses de la regi\u00f3n donde hab\u00eda espacio para desarrollar y brindar servicios de salud de alta calidad, decant\u00e1ndose por el nuestro. Fue as\u00ed como en 2013 adquiri\u00f3 el 40% de Red Salud UC, conformando la Red Salud UC CHRISTUS. Luego, se hizo del 50% de la propiedad de la Cl\u00ednica UC San Carlos de Apoquindo, tras comprar su participaci\u00f3n a Isapre Colmena.<\/p>\n

Un mayor acceso a las distintas formas de enfrentar los desaf\u00edos en salud en otras partes del mundo, por ejemplo incorporando de forma m\u00e1s r\u00e1pida las innovaciones, pr\u00e1cticas y tecnolog\u00eda, es uno de los beneficios que han tra\u00eddo estos movimientos, de acuerdo con el gerente de Desarrollo de Nuevos Negocios de la Red de Salud UC CHRISTUS, Thomas Leisewitz.<\/p>\n

Adem\u00e1s, les ha permitido compararse con la regi\u00f3n y con la realidad del sector salud norteamericano. \u201cLo que va a pasar tambi\u00e9n con otros prestadores que se puedan asociar con grandes organizaciones internacionales. Esto hace m\u00e1s din\u00e1mica la competencia y genera un espacio interesante para mejorar la calidad de los servicios hacia los pacientes\u201d, afirma.<\/p>\n

Leisewitz destaca, asimismo, el \u00e9nfasis en los resultados en la salud del paciente que trajo CHRISTUS, lo que se observa en el cumplimiento de est\u00e1ndares de calidad y eficiencia.<\/p>\n

En ese sentido, acota que el acceso a los servicios es s\u00f3lo el primer paso, pues lo m\u00e1s relevante es que agreguen valor al paciente y vayan realmente en su beneficio, lo que se puede lograr al alinear los incentivos entre el paciente, el prestador y el asegurador.<\/p>\n

Y como la innovaci\u00f3n y la mejora continua son parte de las pr\u00e1cticas sistem\u00e1ticas que trajo CHRISTUS, la red prepara una serie de avances que incluyen mejorar los sistemas inform\u00e1ticos para integrar la informaci\u00f3n cl\u00ednica de los pacientes y que puedan interactuar de manera m\u00e1s eficiente con el equipo cl\u00ednico. Adem\u00e1s, proyectan crecer en el \u00e1rea ambulatoria y desarrollar modelos innovadores en la forma de atenci\u00f3n, ofreciendo un mejor acceso a los centros.<\/p>\n

\u201cDefinitivamente, la colaboraci\u00f3n integral entre ambos pa\u00edses es importante dado el intercambio en materia de investigaci\u00f3n que tambi\u00e9n est\u00e1 aportando Chile en este momento y que, sin duda, puede encontrar para ambos mejores perspectivas a futuro\u201d, concluye Ordo\u00f1ez.\u00a0<\/strong><\/p>\n

Espacios por abarcar<\/strong><\/p>\n

Cl\u00ednica Alemana, luego de su acuerdo con Cl\u00ednica Mayo, suscribi\u00f3 convenios con diversos centros de Estados Unidos, incluyendo The Cleveland Clinic Foundation, The Children\u2019s Hospital of Philadelphia, MD Anderson Cancer Center, Hospital for Special Surgery, Chest Disease Center – Beth Israel Deaconess Medical Center y Nicklaus Children\u2019s Hospital. Adem\u00e1s, tiene relaci\u00f3n con hospitales de Alemania, B\u00e9lgica, Argentina, Italia y Espa\u00f1a. Y es que, dado el competitivo sistema de salud que tiene el pa\u00eds, todav\u00eda quedan enormes oportunidades por explorar.<\/p>\n

Seg\u00fan Bruno Indelli, Managing Director de MSD Chile, la atenci\u00f3n local est\u00e1 sobre el promedio al compararla con Latinoam\u00e9rica, pero frente a la OCDE, todav\u00eda hay campo para avanzar. A su juicio, los desaf\u00edos del pa\u00eds se relacionan con el acceso a terapias m\u00e1s modernas, adem\u00e1s de la falta de especialistas y camas en los hospitales p\u00fablicos.<\/p>\n

\u201cEl intercambio de mejores pr\u00e1cticas con socios estadounidenses probablemente mejorar\u00e1 el nivel de servicio de los proveedores de atenci\u00f3n m\u00e9dica en Chile\u201d, se\u00f1ala Indelli, pero aclara que el impacto es dif\u00edcil de medir, dadas las importantes diferencias entre ambos sistemas de salud y los<\/p>\n

vol\u00famenes del mercado de Estados Unidos, que seguramente aprovechan un poder de negociaci\u00f3n mayor que el que tienen los centros chilenos.[:en]For a number of years, certain Chilean clinics have been working with leading American hospitals to train medical staff and develop a collaborative working relationship in patient care. Private US funds have even been invested in local healthcare centers. According to industry experts, these practices could have a significant impact on improving patient care in Chile.\u00a0<\/strong><\/p>\n

The final few months of 2017 marked the 20th anniversary of one of the longest standing agreements between a Chilean and US healthcare center. The agreement in question is the one signed between Cl\u00ednica Alemana and Mayo Clinic, which currently tops the annual U.S. News Best Hospitals & World Report 2017-2018 ranking of leading hospitals in the United States.<\/p>\n

A separate but similar agreement has also been entered into between Cl\u00ednica Universidad de los Andes and Massachusetts General Hospital, which sits in fourth place in the aforementioned ranking. Likewise, in 2013 the US-based CHRISTUS Health network also invested in the Red de Salud UC network which forms part of the Pontificia Universidad Cat\u00f3lica\u00a0de\u00a0Chile.<\/p>\n

These developments have been inspired by a spirit of collaboration, based on shared values and outlooks, and have directly impacted on the standards of Chilean healthcare centers, with resultant benefits for patients.<\/p>\n

The Commercial Director of Cl\u00ednica Universidad de los Andes, Arturo Z\u00fa\u00f1iga, compares this situation to Chile joining the Organisation for Economic Co-operation and Development (OECD), at which point the country ceased to compare itself with its Latin American neighbors.<\/p>\n

At that juncture, he states, \u201cwe began to see that our productivity, per capita income, level of education and a whole host of other international indicators had progressed significantly and, as a result, the country had achieved more positive outcomes. In comparative terms regarding health, it is a good idea to replicate the best practices of internationally renowned healthcare institutions\u201d.<\/p>\n

According to the General Manager of Sanofi, Henry Ordo\u00f1ez, Chile possesses the technological capabilities and professionals to deliver superior healthcare compared to its Latin American counterparts. However, he stresses the need to improve certain processes to streamline efficiency in resource investment, as well as closer monitoring of those resources in order to identify not only short-term, but also medium- and long-term answers. All of which could be framed within a context of cross-cutting initiatives that aim to enhance care and expand coverage.<\/p>\n

Therefore, the significance of these types of agreements goes beyond just possible investment opportunities and relates more to the improvement of procedures or standardization of protocols, which gives rise to improved ways of comparing local standards to international standards.<\/p>\n

\"\"<\/p>\n

Consolidated collaboration<\/strong><\/p>\n

The scientific-teaching collaboration agreement signed two decades ago between Cl\u00ednica Alemana and Mayo Clinic arose out of the interest of the Chilean entity to forge links with its American counterpart, one of the most prestigious hospitals in the United States, with over 150 years of history and experience in the field of medicine.<\/p>\n

The similarity in values and principles of the two institutions was also a contributory factor behind the agreement, according to the Jefe del Departamento de Medicina Interna (Head of the Internal Medicine Department) of Cl\u00ednica Alemana, Dr Luis Miguel Noriega. The main similarity in this regard is the common outlook of both entities whereby the patient is viewed as the core element within the establishment, and that the establishment, in turn, assumes the duty of medical education.<\/p>\n

\u201cThis exchange of knowledge and search for new information has a significant and mutual value. We and our colleagues in Chile are working to advance medicine in the United States, Chile and the wider world\u201d, says Dr Alfredo Qui\u00f1ones-Hinojosa, Head of Neurology at Mayo Clinic in Jacksonville (Florida) and also President Consultant for projects and initiatives in Latin America for the same institution.<\/p>\n

The relationship between Mayo Clinic and Chile began more than a century ago when the cofounder of the American group, Dr William Mayo, visited the country and formed working relations with a number of local doctors and hospitals. However, the idea to formalize this particular agreement was born out of another visit, this time by a group of Mayo Clinic directors, to Latin America.<\/p>\n

During that trip, Dr Claus Krebs, then-Director M\u00e9dico (Medical Director) at Cl\u00ednica Alemana initiated contact. Soon after, and thanks to the support of Dr Salvador \u00c1lvarez, who was then in charge of international affairs at the US clinic, the introductions had been transformed into a formal agreement. One of the most noted initiatives arising out of the agreement has been the Actualizaci\u00f3n del Board de Medicina Interna (Internal Medicine Board Review) program. This particular scheme has been jointly implemented since 2005 on a biannual basis and its objective is to prepare Chilean doctors for their board certification and recertification exam in the United States.<\/p>\n

The institutional and personal ties that have been created over 20 years of collaboration have, explains Dr Noriega, enabled Chilean patients to receive second opinions from medical professionals at the American clinic. Likewise, Chilean patients have been introduced to medical teams from Mayo Clinic to receive diagnostic and therapeutic assessments and suggestions.<\/p>\n

In turn, professionals from Cl\u00ednica Alemana have visited the United States to learn about the models applied in that country and to gauge their applicability to the Chilean market. For example, one of the most significant aspects in this regard was the implementation of electronic records in the Chilean entity. This came about due to the fact that numerous professionals, whether doctors or individuals from other fields, were able to experience how these worked in practice during their \u00a0trips to Mayo Clinic, witnessing first-hand the associated problems and benefits of their use.<\/p>\n

\u201cMayo Clinic has opened its doors to show us its patient care model, its different areas of work, its mistakes and its successes\u201d, explains Dr Noriega. \u201cThis has significantly impacted certain aspects of our decision-making in terms of strategic planning\u201d, he adds.<\/p>\n

\"\"<\/p>\n

Second opinion<\/strong><\/p>\n

One of the most recent mutual agreements entered into by a Chilean and US health provider is the aforementioned arrangement announced in October 2017 between Cl\u00ednica Universidad de Los Andes and Massachusetts General Hospital (Mass General), the original university hospital of Harvard University.<\/p>\n

According to the Director Comercial (Commercial Director) of Cl\u00ednica Universidad de los Andes, it was representatives of Mass General who initially approached the Chilean institution, on the basis that both healthcare entities are nonprofit clinical campuses in which research and teaching are key priorities.<\/p>\n

The agreement enables Chilean patients to request their medical records to be sent to Boston for review by doctors from Mass General, in order to obtain a second opinion. Within just weeks of this process getting off the ground, more than 10 patients with complex conditions have already requested the use of this service. Their medical records are currently undergoing review by a multidisciplinary team of specialists in order to determine whether they should be flown to the United States for further treatment.<\/p>\n

With regard to the Chilean specialists, the agreement enables them to interact with global leaders in the field from a diverse range of specialties, including the opportunity to share techniques and exchange medical opinions.<\/p>\n

\u201cThe biggest impact that institutions like Mass General can have on Chile is to further strengthen the country\u2019s healthcare systems, in addition to supporting doctors to provide the highest quality of care to patients at the local level\u201d, says the Chief Marketing Officer and Senior Director for International and Specialized Healthcare Services at Massachusetts General Hospital, Misty Hathaway.<\/p>\n

In her opinion, Chile has a robust system of healthcare with top-class clinics and hospitals. However, in line with many other countries, certain disorders are growing in prevalence among the Chilean population, including stomach and lung cancer, kidney disease and diabetes, and these are the consequences of lifestyle choices, diet and environmental factors. As a result, experts from Mass General see this as an opportunity for Chilean doctors and other healthcare professionals to improve education in terms of risk factors. \u201cNo country has a perfect healthcare system, not even the United States!\u201d Hathaway points out.<\/p>\n

Innovation and standards<\/strong>\u00a0<\/strong><\/p>\n

General nationwide conditions, a stable economy, and the opportunity to form a partnership with a like-minded institution in terms of mission and values were the main reasons that attracted CHRISTUS Health to Chile, within a wider context of its strategic objective to position itself in Latin America.<\/p>\n

The Texas-based healthcare center, which forms part of the Sisters of Charity of the Incarnate Word, had already established operations in Mexico prior to its venture into Chile. These moves into Latin American markets fit with the institution\u2019s goal to identify countries in the region where it is possible to grow and provide high-quality care services. In terms of its expansion into Chile, in 2013 CHRISTUS Health acquired a 40% stake in Red Salud UC, with the new entity subsequently becoming Red Salud UC CHRISTUS. Later, the US entity acquired 50% of the Cl\u00ednica UC San Carlos de Apoquindo, having purchased that institution\u2019s stake in the Isapre (private healthcare insurance provider), Colmena.<\/p>\n

The Gerente de Desarrollo de Nuevos Negocios (New Business Development Manager) at Red de Salud UC CHRISTUS, Thomas Leisewitz, believes that one of the key benefits brought to Chile by US healthcare centers, such as CHRISTUS Health, is a greater exposure to different ways in which health-related challenges are confronted in other parts of the world. Accordingly, he explains that access to services is merely the first step, and that the most important aspect is that these services add value to and genuinely benefit the patient. This, he points out, can be achieved by aligning incentives between the patient, the provider and the insurer.<\/p>\n

The Chilean healthcare provider also has agreements with a number of hospitals in Germany, Belgium, Argentina, Italy and Spain. And thanks to the competitive system of healthcare that exists in Chile, there are still many new opportunities to explore.<\/p>\n

According to Bruno Indelli, Managing Director of MSD Chile, while local care exceeds the Latin American average, there is still room for improvement in terms of comparisons with other OECD nations. In his opinion, the major pending challenges in Chile relate to access to modern care, as well as a lack of specialists and beds in public hospitals.<\/p>\n
